Job Summary:
The Geriatric Nurse is responsible for providing specialized nursing care to elderly patients in a variety of healthcare settings. This role requires extensive knowledge and experience in geriatric nursing practices, as well as the ability to effectively communicate and collaborate with interdisciplinary healthcare teams. The Geriatric Nurse will be responsible for managing and addressing the unique physical, emotional, and psychological needs of elderly patients, ensuring their safety, comfort, and well-being.
Key Responsibilities:
1. Conduct comprehensive assessments of elderly patients' physical, cognitive, and psychosocial status.
2. Develop and implement individualized care plans based on the assessment findings, ensuring that the specific needs of each patient are addressed.
3. Administer medications, treatments, and procedures as prescribed, adhering to established protocols and guidelines.
4. Monitor and evaluate patients' response to treatments and interventions, documenting changes, and making necessary adjustments to the care plan.
5. Collaborate with interdisciplinary healthcare teams to ensure coordinated and holistic care for elderly patients.
6. Provide education and support to patients and their families regarding geriatric conditions, treatment options, and self-care techniques.
7. Monitor and maintain patients' medical records, ensuring accuracy and confidentiality.
8. Assist in the coordination of specialized geriatric services, such as physical therapy, occupational therapy, and mental health counseling.
9. Promote a safe and comfortable environment for elderly patients, identifying and addressing potential hazards or risks.
10. Stay up-to-date with current advancements and best practices in geriatric nursing through continuous professional development.
Required Skills and Qualifications:
1. Valid and current nursing license in the country/state of practice.
2. Bachelor's degree in Nursing (BSN) or equivalent degree from an accredited nursing program.
3. Proven experience working as a registered nurse in geriatric care, preferably in a hospital, long-term care facility, or specialized geriatric unit.
4. In-depth knowledge of geriatric conditions, diseases, and syndromes, and their impact on physical, cognitive, and psychosocial well-being of elderly patients.
5. Proficient in conducting comprehensive geriatric assessments, including physical, cognitive, and psychosocial evaluations.
6. Strong understanding of geriatric pharmacology, including common medications, potential side effects, and interactions.
7. Excellent communication and interpersonal skills, with the ability to effectively interact and collaborate with patients, families, and interdisciplinary healthcare teams.
8. Ability to prioritize and manage multiple tasks, demonstrating strong organizational skills.
9. Attention to detail and accuracy in documenting patient information and care plans.
10. Demonstrated empathy, compassion, and patience in dealing with elderly patients and their families.
